Wallace is seeking a dedicated Nurse Practitioner to serve as an empaneled primary care provider for new and established patients, delivering comprehensive, relationship-based care in a collaborative, team-based clinic setting.
Key Details:
- Title: Primary Care Provider - Nurse Practitioner.
- Compensation: competitive salary $140,000-$170,000 DOE, language differential available.
- Schedule: Full time, 4 days a week 10 hour days
- Location: Our Rockwood Clinic, 18633 SE Stark Street, Portland, OR

- Team based care environment: Work alongside NPs, MAs, nurses, care coordinators, behavioral health providers, lab staff, and interpreters.
What You’ll Do:
- Serve as an empaneled primary care provider for new and established Wallace patients across the lifespan.
- Provide preventive care, health screenings, chronic disease management, and patient education to support whole-person wellness.
- Perform clinical evaluations, physical exams, and histories.
- Order and interpret diagnostic tests and labs.
- Collaborate with the Care Team to develop longitudinal care plans and coordinate referrals, follow-up, and preventive services across the care continuum.
- Partner closely with RNs, MAs, behavioral health, care coordination, and other interdisciplinary team members to deliver integrated care.
- Maintain high quality documentation, follow up planning, and patient education.
- Maintain a patient-centered approach that advances access, continuity, and quality outcomes for the communities we serve.
What We’re Looking For: This role is ideal for an NP with primary care experience who values relationship-centered, team-based care and is committed to serving diverse communities with compassion and clinical excellence.
Requirements:
- Current, unrestricted NP license in Oregon
- Current, unrestricted DEA license
- Current BLS certificate
- Experience managing chronic conditions, preventive care, and complex primary care needs across diverse patient populations
- Commitment to team based, patient centered care
Preferred:
- Experience in community health, family practice, FQHC, or other outpatient primary care settings.
- Experience supporting Medicaid or higher acuity primary care populations
- Bilingual in Spanish (preferred, but not required)
- Collaborative, team oriented, and grounded in patient centered values
- Able to balance efficiency with thoughtful follow-up and sustained patient relationship-building.
